Pay Rate: $34 - $37/hr; (Negotiable based on experience)

Shift: Evening Shift : 2:45 PM - 11:15 PM

Position Summary:

We are seeking compassionate and reliable LPNs/LVNs to join our team at a Skilled Nursing Facility (SNF) in Bradford, MA. The ideal candidate will deliver high-quality nursing care, ensure patient safety, and collaborate effectively with the healthcare team to promote resident health and well-being.

Key Responsibilities:

• Provide direct nursing care to residents in accordance with care plans and facility policies.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents' health status, vital signs, and responses to treatment.
• Assist in admission, transfer, and discharge of residents.
• Communicate effectively with residents, families, and interdisciplinary team members.
• Ensure compliance with all state, federal, and facility regulations.
• Respond promptly to resident needs and emergencies.
• Supervise and assist Certified Nursing Assistants (CNAs) and other support staff as needed.

Required Qualifications:

• Active LPN/LVN license in the state of Massachusetts (in good standing).
• At least 1 year of nursing experience in a SNF or long-term care facility.
• Current CPR/BLS certification .
• Strong interpersonal and communication skills.
