Job Details:

\n

• Hospice case management position serving the Oshkosh/Green Bay market
\n• Work in community settings including patient homes, family member homes, assisted living facilities, group homes, nursing homes, and hospitals
\n• Remote/Field work with caseload of 13-15 patients
\n• Complete 5-6 visits per 8-hour shift
\n• Inpatient hospice work with 1:6-9 patient ratio depending on shift
\n• Care for hospice patients with life expectancy of 6 months or less
\n• Patient diagnoses include 40% cancer cases and 60% other medical issues
\n• 2 years of RN experience required
\n• First-time travelers accepted
\n• Required certifications: BLS and state license
\n• Must have IV skills, chest tube maintenance/pleurex drain experience
\n• Perform catheter insertion, tube feedings, and wound care including wound vac
\n• Work with PIV, midline, and central lines
\n• Provide end-of-life care including pain and symptom management
\n• Experience with opioids and opioid equinalgesic table required
\n• Document on EMR platform
\n• Must be independent critical thinker while knowing when to seek assistance
\n• Flexibility required for various care settings
